This location closed the following month after I chose to stop seeing patients in San Luis Obispo on November 27, 2013.

From 101 North: Turn right onto the exit ramp and then turn left onto California Blvd.

From 101 South: Turn left onto California Blvd.

To find the Phillips Lane office, continue on California Blvd. and turn south onto Phillips Lane which is located on the same side of California Blvd. as the CHP building. The office complex is located at the end of the cul-de-sac. Suite 201 is located at the south end of the walkway on the second floor. There are stairs located on either side. An elevator is available as well.
